In conclusion I've decided that it's best to leave Quarterpel and Global Motion Compensation off. Then turn on VHQ mode - I've had damn good results from level 3. Also I've enabled Chroma Motion and enabled B-Frames, so Maximum B-Frames needs raising from -1 to 3. These settings make encoding quite slow but the results are brilliant.
